People that have Carpal Tunnel or Tendinitis will often have trouble writing with pens or pencils because of the strain on the tendons caused by the small grip of the object. When going through physical therapy, patients often use pens designed to be easier to hold. They are often much larger than regular pens and have grooves that make it easier to hold. This is something I thought of because I went through physical therapy for Tendinitis when I was younger.
