Use this form to submit paperwork for a guest speaker honorarium.

For Fall classes, paperwork must be filed by November 15. For Spring classes, paperwork must be filed by April 15. Speaking engagements can fall after these dates, but within the given semester’s academic calendar.

Honorarium Paperwork Submissions

  • Maximum $150, and subject to total class budget constraints.
  • Please ensure that you are submitting ALL required paperwork in a single file. This includes:
    1. Honorarium Service Claim Form
    2. Employee Determination Sheet
    3. Determination of Residency Status
    4. NYS Vendor Registration (if the payee is already registered, include a sheet of paper with their name, mailing address, and vendor number)
    5. NYS Standard Voucher
